Appointments
Appointments should be made in advance by calling the office
location at which you wish to be
seen. All appropriate forms must
be filled out and signed prior to seeing the medical provider.
These forms can be printed from this
site and brought with you to expedite the registration process.
If you have insurance, you must bring all appropriate
documentation and member cards.
Walk-in Appointments
All Med never turns away a patient if there is a physician or
therapist available when you arrive. We cannot help you,
however, if the attending physician is not at the location at
which you are seeking treatment...for instance, the office hours
are ended and the provider has already gone home or left the
clinic.
Cancellations and No Shows
Your scheduled appointment will be confirmed twenty-four
hours prior to your expected visit. Please let our staff
know if you will not be available for your scheduled appointment
as soon as you know you cannot or have decided not to see the
scheduled provider.

filled out online and then printed and/or submitted to our
office via the internet. We have provided some of these
forms in the languages that are most often used by our patients:
Please check the insurance page if this will be your first
visit to All Med in order to assure that you will have the
appropriate paperwork with you when you arrive for your
appointment.
Please read the HIPAA policy. Print it and sign it (if
you wish to save time when you arrive for your first visit) and
bring the signed form with you
